background and aims: The rapid advancement of transformative technologies such as the Internet of Things (IoT), artificial intelligence (AI), cloud computing, and big data has redefined the role of hospitals and led to the emergence of a new generation of medical facilities known as smart hospitals. This transformation extends beyond the technological layer, significantly impacting architectural design and physical infrastructure. The present study aims to conduct a comparative analysis of trends, design patterns, and smart infrastructure in hospital architecture across selected developed countries. Methods: This systematic review was conducted in April 2025 using keywords such as "Smart Hospital", "Architecture", "Facility Design", "Healthcare 4.0", and "Developed countries" across reputable scientific databases including Embase, Web of Science, Scopus, and PubMed. The study followed PRISMA guidelines for systematic reviews. Articles focusing on smart hospital architecture published between 2019 and 2025, with full-text availability in English, were included as eligibility criteria. Result: After screening and quality assessment, 22 selected articles were included in the final analysis. The reviewed models emphasized multilayered design frameworks Sensing, Networking, Remote Services, Knowledge, Application layers. Key enabling technologies such as IoT, cloud computing, Extended Reality, Big Data, Building Information Modeling (BIM), digital twins, and machine learning algorithms played a pivotal role in shaping these layers. Modular, flexible, human-centered, and sustainable design approaches were highlighted as responses to demographic shifts, chronic diseases, and pandemic crises. The influence of environmental elements—such as light, sound, color, and spatial form—on treatment effectiveness and patient satisfaction was explored through novel concepts like psychophysiological responses to space (placebo and nocebo effects). Some countries have moved closer to realizing these models through public-private partnerships, performance-based payment systems, and entrepreneurial digital strategies. Conclusion: Smart hospital architecture emerges at the intersection of technology, physical design, human experience, and health policy. This study presents a multidimensional perspective on the requirements for future-oriented hospital design, emphasizing interoperability, safety, sustainability, and human-centeredness.
