Background and aims: As the IoT technology is progressing, wearable devices prove to be essential in chronic disease management. Real-time monitoring through these devices allows patients to monitor their health status outside the hospital environment. The aim of this study is to evaluate how such devices are effective for improving treatment outcomes and quality of life of patients suffering from chronic diseases. The hypothesis of the study is stating that such IoT based wearables might lower the occurrence of complications of the disease and improve symptoms control. Method: This study was conducted as a review in 2025 and included searches in reputable databases such as PubMed, Web of Science, Scopus, and Google Scholar search engine. Keywords including "Internet of Things", "wearable devices", "chronic disease management", "health monitoring", and "remote monitoring" were reviewed in studies without time limits. In the initial search, 102articles were found and after screening, 45 articles were selected for further analysis. Results: The present study claims that IoT-based wearable devices significantly improve the management of chronic diseases. If these devices allow for real-time monitoring of patients' health status, then they could afford dramatic cost savings and prevention of disease exacerbation as well as facilitate treatment. Such wearable devices provide valuable clinical information such as blood sugar levels, blood pressure, physical activity levels, and heart rate such that doctors can prescribe better treatment modalities. These technologies may also prevent hospitalizations and complications associated with chronic diseases. The use of such devices does not only help improve the management of diseases but also increases patient satisfaction and the quality of life. Conclusion: Advancement in wearable technologies and IoT enable continuous monitoring of patients' health status. These can prevent disease exacerbation, reduce costs, and improve the quality of life of the patient. Privacy and data security issues must be given attention. Future research should focus on developing predictive models and studying long-term effects of these technologies on disease management.
