Background and aims: The emergence of technological innovations and digital health is creating fundamental changes in the healthcare sector, towards personalized medicine. Therefore, it is essential to identify the appropriate science and technology policy for the transition to this disruptive technological paradigm and its implementation. The aim of this study is to identify and prioritizing the appropriate science and technology policies for the implementation of personalized medicine in Iran. Methods: Using an expert panel and survey with an AHP questionnaire, appropriate policies for implementing personalized medicine in Iran were prioritized, and then the best policy for its implementation was determined. The statistical population included physicians, geneticists, pharmacists, nutritionists, proteomics, biochemists, molecular cell biology, and software and artificial intelligence specialists who were also experts in personalized medicine and had at least 3 years of experience (on average) in personalized medicine. The sample included 26 people. Results: The results show that Iran is in the pre-development phase of personalized medicine, Currently and at this phase, the best policy is to regulation and infrastructure development, including the launch of genomic databases, artificial intelligence development, and big data analysis from electronic health records. Also, the best mechanisms for developing personalized medicine are “employing foreign experts”, reforming processes by the government and considering businesses without government intervention, customizing research by monitoring a working group in the Ministry of Health, allocating budgets to practitioners’ research and training, developing a PM program based on the existing structure and experiences of different countries, financing and creating incentives for startups, allocating sufficient resources to research centers, and enhancing the connection between industry and universities. Conclusion: The most important technological and innovative policies at different phases of personalized medicine development in Iran respectively include “infrastructures development and regulation”, “stimulating the innovation demand side”, and “stimulating the innovation supply side”, respectively.
