Background and Aims: The Internet of Things (IoT), by connecting devices to the internet and enabling real-time data sharing, has significantly transformed personalized medicine. Personalized healthcare aims to tailor medical care to individual genetic, environmental, and lifestyle factors. IoT plays a crucial role in this transformation by enhancing monitoring, customizing treatments, and improving patient outcomes. Method: To investigate the role of IoT in personalized medicine, a systematic literature review was conducted. Major academic databases including PubMed, Scopus, Web of Science, and IEEE Xplore were searched. The search covered the period from January 2015 to December 2023. Keywords used included “Internet of Things,” “IoT,” “personalized medicine,” “wearable devices,” “remote monitoring,” and “healthcare IoT.” After screening titles and abstracts, duplicate records were removed, and full texts of relevant articles were assessed. A total of 235 initial records were identified, out of which 42 studies met the inclusion criteria and were selected for final analysis. Results: Among the 42 selected articles, 78% focused on the use of wearable devices for continuous monitoring of vital signs such as heart rate, blood pressure, and glucose levels. Predictive data analytics for early detection of diseases like cardiovascular conditions and diabetes were discussed in 65% of the studies. Smart pillboxes and adherence-promoting technologies were featured in 53% of the articles. Additionally, 48% of the reviewed studies highlighted the importance of real-time data sharing among healthcare providers to enhance care coordination. Overall, the findings demonstrate that IoT significantly contributes to improved therapeutic outcomes by enabling timely, data-driven, and patient-centered care. Conclusion: IoT has revolutionized personalized medicine by delivering real-time insights, customized care strategies, and enhanced patient engagement. Despite existing challenges such as data privacy concerns, system integration issues, and device reliability, IoT remains a transformative force in the future of healthcare delivery
