Background and aims: Health Information Systems (HIS) are vital tools that help in the collection, processing, and distribution of information, clinical, and management decision-making.Successful implementation of these systems has always been accompanied by challenges. Studies show that a significant percentage of HIS projects fail, which can be due to technical, organizational, human, or environmental factors. Therefore, identifying these factors is essentialto improve the success of health information systems. This review article comprehensively examines the factors affecting the success and failure of HIS. Method:This study was conducted as a systematic review.To collect data, reputable databases including PubMed, Scopus, Web of Science, and Google Scholar were searched. Keywords used included "Health Information Systems", "Success Factors", "Failure Factors", "Technical Factors", "Organizational Factors", "Human Factors", and "Environmental Factors". Articles published between 2015 and 2023 in English were reviewed. After initial screening, relevant articles were selected and their data were extracted and analyzed. Results:Based on the analysis of studies, the factors affecting the success and failure of health information systems are divided into four main categories: 1.Technical factors: hardware and software quality,system integration with other existing systems, information security and privacy, system reliability and stability 2.Organizational factors: senior management support, the existence of appropriate strategy and planning, allocation of sufficient resources, organizational culture and acceptance of change 3.Human factors: user skills and knowledge, user participation in system design and implementation, user training and support, resistance to change 4.Environmental factors: government laws and regulations, competitive pressure and stakeholder expectations, economic and political conditions Conclusion:The success of health information systems is significantly influenced by technical, organizational, human, and environmental factors. To increase the likelihood of success, organizations should pay attention to these factors and have a comprehensive plan for managing them. Top management support, user training, allocation of adequate resources, and attention to information security are key measures. Also, understanding the needs and expectations of stakeholders and adapting the system to environmental conditions can help improve HIS performance. Future studies can examine the combined effect of these factors and provide models for predicting the success of health information systems.
