Background and Aims: Artificial intelligence (AI) is increasingly being utilized to predict the success of varicocele surgery in the context of infertility treatment. Varicocele, an enlargement of the veins within the scrotum, can affect sperm quality and male fertility. By leveraging AI algorithms, clinicians can analyze various factors that contribute to surgical outcomes, leading to more personalized treatment plans. Methods: Varicocele may lead to reduction of sperm production and count, disrupt male fertility, alter sperm morphology, and diminish sperm quality. Consequently, in assisted reproductive techniques such as IVF and ICSI, satisfactory results may not be achieved. While varicocele surgery may improve sperm quality, it remains an unresolved issue to determine which patients are suitable candidates for the surgery concerning sperm quality. Results: One of the key applications of AI is in the development of predictive models using both deep learning and machine learning, which incorporate patient-specific data such as age, semen analysis results, and hormonal profiles. A study by Bensalah et al. demonstrated that AI could effectively predict the likelihood of improved semen parameters post-varicocelectomy based on preoperative data. Additionally, AI can assist in identifying biomarkers correlated with surgical success. Research has shown that AI-driven analysis of sperm DNA fragmentation and oxidative stress levels can provide insights into the expected outcomes of varicocele repair. This enables specialists to better stratify patients according to their likelihood of benefiting from the surgery. Conclusion AI plays a crucial role in predicting the success of varicocele surgery in infertility treatment by utilizing data-driven approaches to improve patient selection and optimize surgical outcomes.
