Background: Telehealth is one of the ways to provide healthcare services to all people in society, especially those who live in deprived areas. It has benefits such as lower cost and easier access to high quality services. Policy-making in health is also carried out with the aim of improving and promoting the quality of life of individuals. This policy-making is influenced by various factors, including cultural factors. Aim: This study aimed to examine the impact of cultural factors on policy-making in the field of telehealth in Iran. Materials and methods: In this review, selected keywords including telehealth, telemedicine, culture, Iranian culture, policy, and health policy-making were searched from Persian published articles using Scopus, SID, Cochrane Library, and Google Scholar databases. Articles that met the inclusion criteria were selected and extracted from all of them. Findings: Based on the seven selected articles, the impact of cultural factors on policymaking in telehealth in Iran was divided into eight categories: 1. Weak privacy protection methods, 2. Patients' preference for face-to-face visits, 3. Patients' concern about the quality of services provided, 4. Attitudinal and behavioral limitations of people, 5. Avoiding technology in the elderly, 6. Resistance of doctors and healthcare providers, 7. Language difference, level of health literacy and academic literacy, and 8. The phenomenon of resistance to change. Conclusion: Considering the increasing development of information technology and its application in life, especially in the field of people's health, the successful development of telehealth and solving cultural problems in the country's health system requires increasing the awareness and training of people both as service providers and as recipients. Policymaking in this area should be designed comprehensively and by considering cultural factors, because the close relationship between policymaking in health and culture results in the creation of better and stronger solutions in society.
